As seen above the dues card application form is provided via link, please ensure that after filling out the form that you sign it as well as including the annual dues of $50.00 and two (2) passport sized photos of yourself to be sure the Grand Lodge has them on file.

For the information of the Brethren. Life Membership by Purchase has already been discontinued since 2003. Therefore, LMP brethren will have their payment be pro rated from the year they have purchased life membership. Pass this message to all LMP brethren.
