Brethren only need to fill out a Dues card application if they wish to receive the hard plastic dues card with photo. If you only require the normal paper dues card then you only need to pay your annual dues and verify your current contact information.

For those who wish to receive the hard plastic dues card. Please fill out and submit the dues card application as well as two passport size photos. If you have submitted photos within the last couple of years there is no need to send new ones unless you want a newer picture on the card. This also applies to Life Members who wish to receive the Hard Card. These cards are issued every year.
